Individual Session 50 minutes- $130
Individual therapy is a joint process between myself and you as a client. Common goals of therapy can be to make life changes or improve your quality of life. People may seek therapy for help with issues that are hard to face alone. Individual therapy sessions typically occur once per week, but can also be held every other week, based on presenting needs and concerns.

Family Session 60 minutes (2 People)- $180
Due to the unique and sometimes complex dynamics of families, family sessions involving two people is often necessary. Some examples of the dynamic of a family session involving two people may include siblings, parent and child, parents of both minor and adult children. Two-person sessions can be done in conjunction with individual and other family sessions involving three or more people, or just as stand alone sessions.

Family Session 90 minutes (3 to 6 People)- $250
Family sessions can sometimes involve 3 or more family members that meet to discuss concerns that may impact the entire family unit. Dysfunctional dynamics, interactions, and beliefs are explored and processed to help facilitate more effective ways of interacting with family members. Family sessions can be done in conjunction with individual and two-person sessions throughout the treatment process.

Payment Options
I am currently a provider for Lyra Health for individual sessions for the states of Georgia, Ohio, and South Carolina. All other services are private pay at this time.
You may be eligible for partial service payment reimbursement with out-of-network benefits through your insurance provider. You are responsible for verifying this information with your insurance provider.

What is a DOT Substance Abuse Evaluation?
A DOT substance abuse evaluation is a comprehensive clinical assessment that is completed by a qualified DOT Substance Abuse Professional (SAP) to determine the level of assistance a DOT employee needs in resolving problems associated with alcohol or prohibited drug use. A recommended course of education and/or treatment must be completed successfully prior to returning to DOT safety-sensitive duties. Before DOT Evaluation
Before your DOT evaluation, there is information needed from you for your SAP. This information includes your DER contact information and a copy of your violation report. For FMCSA employees, registration in the Clearinghouse is required. Health insurance coverage and work schedule information will be needed to determine the best treatment options available. You will be required to DOT’s confidentiality limitations and Statement of Understanding, and obtain release of information

Evaluation & Treatment Process
During the evaluation, there will be a review of the DOT return to duty process. There will also be explanation of the SAP role to employee. A comprehensive clinical assessment which includes completing a biopsychosocial and an in-depth substance use and abuse history.
Once the clinical assessment is complete, the SAP will determine which level(s) of care (education and/or treatment) is needed for return to duty. The employee will be given a minimum 5 options within a 50-100 mile geographic area. It will be the employee’s decision to decide which treatment provider they will utilize.

Follow Up Evaluation
During the follow-up evaluation process, the SAP will obtain verbal and written verification of successful completion from treatment provider. The employee is required 2 negative results on independent non-DOT tests before follow up evaluation. The return to duty test process will be discussed. Employee’s level of participation, motivation, and changes in attitudes during treatment will be assessed by the SAP. There will be a review of an abstinence maintenance plan with employee, and possible recommendation for a plan of aftercare if there is concern about employee’s sobriety or abstinence.

Payment
As an SAP, I am currently a provider for Magellan. If you do not use Magellan, payment for services are to be paid in full by DOT employee before any services are initiated. Fees for both an initial and follow-up evaluations are $450, and may be paid using money transfer services such as Zelle or Venmo.

What Is Supervision?
Supervision involves the relationship between licensed supervisor and supervisee that promotes the development of responsibility, skill, knowledge, attitudes, and ethical standards in the practice of clinical social work. The priority in the supervision process is accountability for client care within the parameters and ethical standards of the social work profession. Georgia Supervision
Fees and days for supervision for MSWs and dependently licensed LMSWs are as follows:
$75 per hour for individual private supervision. Sessions are held based on my availability.
$60 per hour for shared individual supervision (2 supervisees); (2 supervisees must be present to pay $60 rate). Sessions are held for one hour on a day and time agreed upon by myself and supervisees.
$50 per hour for group supervision (3 to 6 supervisees). Sessions are held every 1st and 3rd Tuesday.

Ohio Supervision
Fees and days for supervision for MSWs and dependently licensed LSWs are as follows:
$75 per hour for individual private supervision. Sessions are held based on my availability.
$50 per hour for group supervision (2 to 6 supervisees). Sessions are held every 2nd and 4th Tuesday.
